A FACTORY in Widnes is under investigation after a worker was crushed to death by a forklift truck.
Daniel Mahon, aged 44, from Anfield, Liverpool, was killed after becoming trapped under the truck at Merseyside Coatings in Pickerings Road, Ditton, shortly after midnight on July 28.
Fire crews tried in vain to free him using inflatable equipment but despite the efforts of paramedics he died of severe head injuries at Whiston Hospital.
Officers from Cheshire Police's major incident team initially investigated the incident, which has now been taken over by the Health and Safety Executive who is preparing a file for the coroner.
