Starting their second innings 179 runs adrift on day three, Cook was lbw to Mohammad Abbas in the second over.
Cook has now made single-figure scores in four of his last six innings and England face an uphill task to avoid a sixth defeat in eight Tests.
Mark Stoneman and Joe Root battled to take England to 19 for one after 10 overs, still 160 runs away from making Pakistan bat again.
Earlier, Pakistan added 13 to their overnight total before Mark Wood had Abbas caught behind in the day’s fifth over to bowl the tourists for 363.
Babar Azam, who retired hurt for 68 on Friday after being stuck by Ben Stokes, did not return and is set to miss the rest of his side’s tour with a fractured forearm.
