A NEW £28 million fund to help improve health and social care for older and vulnerable people has been launched.


Warrington Borough Council and its health partners have launched the Better Care Fund which will cover a range of health and social care services to try and reduce hospital and care home admissions.


It will also help those being discharged from hospital to reduce the chance they will need further treatment in hospital.


Professor Steven Broomhead, chairman of Warrington’s health and wellbeing board said: “By pooling our resources and joining-up various services we will be able to provide better support to vulnerable people.


“This will help them to carry on living independently in their own homes, where they are connected to their communities, avoiding or delaying the need for them to be taken into hospital or to move into care homes.


“In Warrington, the council and its health partners have worked for a number of years to create more joined-up services which help people to regain independence after a hospital stay and avoid emergency admissions to hospital. The Better Care Fund will help us to strengthen our joint approach.”


The Better Care Fund has been established by Warrington Borough Council and the NHS Warrington Clinical Commissioning Group. The integrated approach is supported by the 5 Boroughs Partnership NHS Foundation Trust, the Bridgewater Community Healthcare NHS Trust and the Warrington and Halton Hospitals NHS Foundation Trust.
