HAZEL JACK Passed away peacefully at the Forget Me Not Unit, Warrington General Hospital on 22nd July 2015 aged 85 years. Will be sadly missed by Alec and family. Funeral service will take place at Christchurch, Padgate on Monday 3rd August 2015 at 2.00pm followed by cremation at Walton Lea Crematorium at 3.00pm. Flowers or donations to the Forget Me Not Unit.
